It takes about five to ten minutes on any phone, tablet, or computer, and it lets your physiotherapist, chiropractor, or massage therapist review your health history before you walk in — so your appointment is spent on assessment and hands-on treatment instead of paperwork at the front desk.
New physiotherapy and chiropractic patients complete the New Patient Consent Form. Massage therapy patients complete the RMT Intake Form. The Electronic Transmission Authorization Form is completed alongside either one.

Before Your Appointment

Please submit at least 24 hours before your appointment where possible. That gives your practitioner time to review your history in advance, so your scheduled time is spent on hands-on assessment and treatment rather than filling out paperwork at the front desk.
What to Expect at Your First Visit
Your first appointment is primarily an assessment. Your physiotherapist, chiropractor, or massage therapist will review the history you provided on your intake form, ask about how your symptoms started and what makes them better or worse, and complete a physical assessment of movement, strength, and the affected joints or soft tissue.
From there, you will get a working explanation of what is driving your symptoms and a proposed treatment plan — including how many visits are typically needed, what will happen in each one, and what you can do at home between appointments. Most patients begin hands-on treatment during that same first visit.

Which intake form do I need to complete?
If this is your first visit for physiotherapy or chiropractic care, complete the New Patient Consent Form. If you are booked for Registered Massage Therapy, complete the RMT Intake Form. The Electronic Transmission Authorization Form is completed in addition, when you consent to your health information being sent electronically. If you are unsure, call us at 647-691-7667 and our team will confirm which form applies.
How long before my appointment should I fill out the form?
Complete your form at least 24 hours before your appointment where possible. This gives your physiotherapist, chiropractor, or massage therapist time to review your health history in advance, so more of your visit can be spent on assessment and treatment rather than paperwork.
How long does the intake form take to complete?
Most patients finish the New Patient Consent Form in about five to ten minutes, and the RMT Intake Form takes a similar amount of time. Having your health card, extended health insurance details, and a list of any current medications nearby will make it faster.
Can I complete the intake form on my phone?
Yes. All forms are mobile-friendly and can be completed on a phone, tablet, or computer — including the electronic signature at the end of the form.
What if I forget to complete my form before arriving?
Please arrive about fifteen minutes early and our front desk team will help you complete it on site. Filling it in ahead of time is still preferred, because it protects more of your scheduled treatment time.
Is my information kept confidential?
Yes. Your intake information is handled as part of your clinical health record and kept confidential in accordance with Ontario’s Personal Health Information Protection Act (PHIPA) and the standards of our regulatory colleges.
Do I need a doctor’s referral before my first appointment?
No referral is required to see a physiotherapist, chiropractor, or registered massage therapist in Ontario. Some extended health insurance plans do require a referral for reimbursement, so it is worth checking your specific plan before your visit.
